Consent Form

My question is about the consent form. I want to use a real life story for the video, and that was actually happened in a doctor office, and it was between my doctor, his nurse, and myself. Do I have to ask them to sign the consent form if non of them appear in my video, and I just create some cartoon characters instead?

  Hi Katherine,

  Thanks for your question. You do not need consent forms for individuals who do not appear in the video. Just be sure not to mention your name, the doctor's name, or the nurse's name in the video.

  Per the Official Rules:
  "The Video should not include any personal identification information about those in the Video (e.g., full name, address, social security number, etc.)."
